I guess I get to pile on this one too. Ubuntu Jaunty.

It's worth noting that PA ticks along fine and I only get the assertion when I launch pavucontrol.

Hi, I have the same error on Jaunty release, i found out that when i have enabled "Allow other machines on the LAN to discover local sound devices" on an older PulseAudio version in the network (namly 0.9.10) i get this error as soon as i enter the volume manager. When I disable this option, it works like a charm...
