Combine volume meters and volume control in one application

Description

When installing Pulseaudio on my system, I get a whopping number of 6 menu items to my Sound & Video applications menu, which seems huge to me for what I would call a technical "sound management" application.

A way to start reducing the number of items is to combine the Playback and Recording volume meter and the Volume control into one application. These three programs are indeed very related to each other, and often used together.

As any meter or control can be connected to any server, it is very important that the server name (and perhaps the IP address) is shown in the application window, for example in the top/above the cotrol.

Many people probably wants to control local settings (or the default server) most of the time, and want to be sure what they are doing when it is something "special" as changing settings on other pc's.

Is it possible to mark (e.g. by color) if the server is local or remote (default or non-default), or a text like "The server runs on an other computer!"?

pavucontrol from git now combines volume control and volume meter.
