pavumeter should display host name for non-local pulseaudio daemons

If pavumeter does not display local, but remote levels, the name of the host on which the pulseaudio daemon should be displayed, i.e. the behaviour should match local/remote window titles in metacity, where the host name is also added for remote hosts.
